Open enrollment is the annual period when individuals can sign up for, change, or renew their health insurance coverage. During this window, you can review your plan options, add or drop dependents, and make updates to ensure your coverage fits your needs for the upcoming year. After open enrollment closes, changes are generally only allowed if you experience a qualifying life event, such as marriage, the birth of a child, divorce, or starting your own business.

Enrollment for New Firms
Eligible firms of any size can enroll online by filling out the questions on the top right of this page. Once you submit the quote request, you will receive the customized proposal within one business day. Once you review the proposal, we are here to help you with questions you may have and assist you in determining which plans best fit the needs of your firm.
If you decide to enroll with CalCPA Health, here are some forms you will need to fill out:
- Enrollment and Change Form – Complete to apply for new coverage, to make changes to an existing policy, or to waive coverage
- Automatic Payment Authorization Form – Complete to enroll in autopay
- Electronic Billing Registration Form – Complete to request online view-only access to invoices/payment history
- 2026 Subscription Agreement for Solos (no W-2 employees)
- 2026 Subscription Agreement for Direct Groups (Groups of 2+ without a broker)
- 2026 Subscription Agreement for Brokered Groups (Groups of 2+ with a broker)
