Renewing Firms

The annual open enrollment period is the time for firms currently participating in the CalCPA Health plans to make administrative changes to your benefits, such as:

Add or remove plan options (medical, dental, vision, life, and disability)
Adjust your new-hire waiting period or minimum-hour requirements
Enroll employees/dependents who previously waived coverage
Terminate active employees/dependents who now wish to waive coverage

2026 Benefit Changes – Large Group / Small Group / Solo

Enrollment and Change Form – Complete to apply for new coverage, to make changes to an existing policy, or to waive coverage

New Firms Enrolling

Eligible firms of any size can enroll online by filling out the questions on the top right of this page. Once you submit the quote request, you will receive the customized proposal within one business day. Once you review the proposal, we are here to help you with questions you may have and help you determine which plans best fit the needs of your firm.

If you decide to enroll with CalCPA Health, here are some forms you will need to fill out:

Enrollment and Change Form – Complete to apply for new coverage, to make changes to an existing policy, or to waive coverage
Automatic Payment Authorization Form – Complete to enroll in autopay
Electronic Billing Registration Form – Complete to request online view-only access to invoices/payment history
2026 Subscription Agreement for Solos (no W-2 employees)
2026 Subscription Agreement for Direct Groups (Groups of 2+ without a broker)
2026 Subscription Agreement for Brokered Groups (Groups of 2+ with a broker)
